By looking at someone he's able to physically hurt them. It's not really fully explained why that is but it seems to depend on his thoughts where the wounds appear and what kind of wounds they are. Maybe he's only unable to control it because he's a pervert for pain. Anyways, he usually wears a bandage, to avoid complications. Though he takes it off whenever he can.
Also, since he's created by a god he's hella overpowered, he won't die, even if you shatter him to pieces. Only special weapons/ abilities can cause his death. He can just regenerate himself by calling back all the lost parts of his body. He usually waits for this as long as he can to enjoy the pain though.
