Gaston is Lionel's slightly younger cousin. 

In Italy, he struggled with a morphine addiction after becoming a doctor, and he got into a pretty bad spot. His cousin Gray, Lionel's older brother, offered to get him his next fix when he was flat-out of money, and used that addiction to basically force Gaston to sleep with him... Lionel is not the worst of his siblings.

Broken, out of money, and ready to die, Gaston ran into Lionel while he was visiting Italy. He immediately took Gaston in and weaned him down off the drug. Gaston is forever grateful to Lionel, serving as the doctor for him and his family without question or care. 

Occasionally he helps Lionel or the boys torture folks in the basement, people who've hurt them or wronged them... like the man who tried to rape Arcanus, or the one who made Josefina miscarry.
