Monroe is an ex-actor turned back-alley surgeon. After a career as a famous actor and an unfortunate death, they decided to dedicate their life to something they truly love. Most of their clientele involve criminals, those who can't afford the expense of an actual hospital, and emergency cases who couldn't get anywhere else. They'll basically accept anything as payment, though their main currencies tend to be cash, food, and body parts/organs (which they can either use for transplants or sell for medical supplies.)
Personality
Monroe is, in short, kind of an asshole. They are naturally very smug and sarcastic, yet they have a shocking sort of charisma to them. Their nihilistic and morbid sense of humor pairs with their outward flippancy about mortality to make them someone who cares very little about getting into trouble. They'll mock or tease people who get aggressive with them. After all, what are they going to do? Kill them? They're incredibly petty and no stranger to revenge if people get on their bad side. They don't mind making threats or coming off as intimidating if they have to. They have very few morals, and considering most of their clientele are hardened criminals evading the law, they think this is for the best. They're apathetic at best to a lot of issues they face, at most making jokes of the worst situations they see. Their days of fame absolutely rubbed off on them as a person. They are INCREDIBLY good at lying and playing roles when they have to. They can craft a story with ease, and are able to talk their way out of just about anything. They have a huge ego and a large desire for attention. These traits cause them to have very dramatic and energetic mannerisms, the kind that can be likened to those of a villain in a musical. This can also be attributed to their habit of listening to music whenever they perform medical procedures, often syncing actions to beats. Alongside these traints, they can get very raunchy and flirtatious if they want to. They love to playfully flirt and tease others, and get very touchy with those who let them. These mannerisms often play into their already dark sense of humor, proving for very... interesting conversations. However, these traits don't inherently make them bad. While they are often an asshole, they are friendly to those who get on their good side (which is easy enough if they're treated decently.) They're not incapable of empathy or love, and even if they won't admit it at first, they're actually quite sweet at heart. They are fiercely loyal to those who get close to them, and would do anything to keep them safe. They are incredibly smart, even if they don't seem like it, and are very good at their job. They can and will save your life, a job which they put the utmost focus and care into. Medical work is their passion, through and through, and they're no stranger to letting you know about it. They'll even get optimistic for you and give you their utmost hope! Even if their idea of hope-giving is... questionable.
Underneath Monroe's outer persona lie their deepest insecurities and flaws. Their days of fame definitely messed them up in many ways. They collected many traits from the roles they played, and have adopted many into their personal life as automatic habits. This can lead them to feeling quite conflicted and confused about themself at times. They have issues with their identity and feeling "real," in a way. For the longest time, they were controlled and forced into a specific mold by those around them. On the one hand, they're very aggressive about being independent and able to decide who they are and what they do for themself. They hate being put under others' authority. However, on the other, they don't entirely know what they want out of that independence. They have obvious things of course, and overall like the life they made for themself, but sometimes they question whether the person they've decided to become is the "right" one. They also find themself craving approval deep down. Much of their first life was spent pleasing people around them, at the risk of being punished if they failed. They fear that others will treat them the same way. They want to be loved, they want to be wanted and be desirable. They want to be enough just the way they are, even if they themself are unsure of the "self" they've created. They crave attention and fear abandonment. They hate that they feel that way, and to some extent it's part of why they can portray themself in such a hostile way at first. As much as they desire close attachments and love, they want to avoid the worries of pleasing others by making the only one they have to please be themself, in a sense. They have huge issues with their energy. While they project themselves as being a rather eccentric person, they honestly don't have a ton of energy to speak of. What they project is often their highly-caffeinated self. In all actuality, they are very tired. If left without caffeine, they will get pretty grumpy pretty quick. They're fast to lash out if they aren't allowed to either sleep or recaffinate (though they do apologize for tired mood swings.) Due to their drug use during their acting days, they have some lasting mental side effects. These include episodes of paranoia, panic attacks, and hallucinations (mainly tactile, though sometimes audio or visual.) They hate to draw attention to these, but they do detrimentally affect their life. They often open up and accept comfort from those closest to them, but that takes a while to say the least. They have many regrets from their first life, such as their drug use, but they overall see their current situation as an improvement. No matter what horrors they have to face, they're truly free for the first time. Even if they know they can't die outside of very specific circumstances, a part of them truly fears facing death again one day. They don't want to let anyone know that fact, but it's truer than one may expect.
Backstory

Monroe was born into a very wealthy family. Both of their parents were famous actors, loved by the public but very narcissistic behind the curtain. Their relationship wasn't one made out of love, but more out of a desire to boost both of their popularity. They saw Monroe less as a child and more as a means to make them more money and fame—a way to boost their ego, in a sense. They wanted their child to become an acting star just as they had in order to fulfill this goal. Monroe, however, never had an interest in being on screen. They were obsessed with anatomy and the medical field from a young age, and desperately wanted to become a doctor when they grew up. Specifically a surgeon, but they weren't exactly picky on the specifics. They exhibited odd mannerisms due to this obsession; while they never hurt anyone, much of their life still revolved around the subject. Just about any conversation had with them would eventually evolve into one about their interests. They loved to talk about medical procedures, biology, anatomy, and all manner of diseases and injuries. They often played medical-themed games of pretend, or performed "surgeries" on broken toys to fix them. They didn't shy away from even the most macabre and gruesome sides of the medical world, and were instead fascinated by them. This proved to be unsettling to others, but Monroe didn't mind. They loved their passion and wanted everyone to know it.
Their parents, fearing for their own reputation and the prospect of losing the opportunities they wanted for their child, wouldn't accept that. They took to being very controlling and abusive in order to get Monroe to do as they were told. They gave Monroe constant monitoring, managing every tiny facet of their life to make them the perfect star. They forced them to dress and wear their hair in specific ways, eat curated meals and do strenuous activity to keep them in "peak form," say and do certain things to make them as charming as possible, and more. If they disobeyed in even small ways, such as wearing different outfits, eating different things than were picked for them, or letting their true interests slip into conversation, they were harshly punished. They quickly became terrified to disobey, caving to their parents' demands. They let their parents push them into various auditions and activities. They were forced to see and deal with the filthy underbelly of the entertainment world. They hated it, but it did give the results their parents were after once Monroe landed their first small role in their early teens. Popularity came to them quickly as they became a part of more and more media, always being under strict supervision by their parents and the teams surrounding them. However, despite the constant controlling nature of those around them, they never truly gave up on that dream of theirs. They would still study their interests in private. When they were in their early twenties, they received their first leading role: they were to play the part of a head surgeon in a medical TV show that prided itself on its accuracy. Of course it was overdramatized for entertainment, but the science and procedures weren't. It was as close to Monroe's dream as they had ever gotten. While they were on set, they felt like the operations they were doing could easily be real. It was their best role yet, and even if they were still crushed under the controlling heel of those around them, they could still find a bit of happiness in being able to play surgeon. Little did they know how far downhill they could go.
Soon after receiving the role and getting good reception, Monroe got hooked on drugs. The highs were incredible to them, pushing them to heights and energy levels they could hardly dream of. It gave them something to focus on and cope with, despite how destructive it was to them. The withdrawals were painful, often full of horrible mental and physical symptoms. It wasn't uncommon for them to nurse themself through bouts of sickness from it. They were able to hide their addiction rather well, knowing just how deeply punished they would be if it ever slipped out, but eventually it got out of hand. The dosing they needed kept increasing to dangerous levels. The effects kept getting worse and more noticeable. When they were high, it was easy for the lines between their real life and their acting to blur. It all came to a head with one incident. While they were high on their way to work, they passed by a man on the side of the road in need of medical attention. They decided to take the man on set and operate for real during that take. By some miracle, they didn't kill their patient. The procedure was surprisingly successful given the situation, in fact. However, they did mortify their coworkers and attract the attention of the police. Before they were able to be arrested however, their drugs caught up to them. They had overdosed unknowingly before finding their "patient," and ended up dying. Little did they know, that death wouldn't last them very long.
One moment, they were being examined by a coroner and placed in the morgue. The next, they were sitting on a lab table, alive and mostly-well. Monroe didn't quite know what had happened, but they wasted no time in embracing their second chance. They escaped from the lab and made their way back into the world with a few differences. The first thing they realized was a startlingly high level of exhaustion. They were quick to pick up caffeine in order to cope. The next was their dulled sense of pain. Stab wounds felt like papercuts and bone-breaking blows felt like stubbed toes to them. The third was their seeming immortality: no matter what ailment afflicted them, they were able to thrive through it. They had, essentially, been turned undead. These changes shook them to their core at first... but then they realized the potential they had been given. The person they were before was legally dead. For the first time in their life, they were free from the controlling hands that had kept them in line for so long. They could do whatever they wanted. BE whatever they wanted. Upon that realization, they wasted no time embracing their second chance to do what they had always wanted to: be a doctor. Due to their circumstances (what with being legally dead, and otherwise likely having a warrant out for their arrest at best should anyone learn what happened), they knew they wouldn't be able to attend medical school. Instead, they decided to become a back-alley surgeon—a good one at that. To this day, they still work in their back-alley, perfectly happy with the life they've created with their second chance.
