Her shoulders and arms are made from dead tree bark, her hair is a faded moss-like substance, and has a hole in her forehead with a whispey hair-like thing coming out. Her face is almost always stuck to a constant sad eye-d expression, due to her mourning and sadness she used to feel when she first became an entity. She has since gotten used to her purpose.
Her ability allows her to separate a person's soul from their body with a scythe, or she can walk with a person's soul to convince them to pass on before the soul turns into a whisp and floats up into space. Sometimes the process of a person's soul never turns into a whisp to move on, only when a person is too stubborn enough to pass on.
She is a grounded being in a literal sense and a specter-like entity on earth. The only time when someone can see her is when a person is about to or in the middle of passing away. Due to the person's body being separated from the soul, the brain hallucinates and sees Grim as someone or something else. It's very rare when someone sees Grim in her true form when they are dying.
