Basically there's war! and bc of the trauma of it's opening weeks, Ichigo disappeared for like 8 years, isolating himself from his found family, and society as a whole.
he succumbed to guilt and depression, as he blames the whole war on his failure during Operation: The Fall (which was meant to prevent the war from happening,
but their enemy overwhelmed them and so they lost it).
After those 8 years, his old friends find him, and convince him to return to their family unit and to join them at their shelter,
which is a relatively big underground village where they hide from the enemy.
His story focuses on him learning to socialize again, to menage his crumbling mental health, and to forgive himself.
During The Fall he sustained a nasty injury to his ankle, which can cause debilitating pain to this day! He usually ignores it and his limitations,
because he's like, known as a good runner and feels that's the only thing he's ever been good at.
Suicide/Self Harm Mention:
The wrist scar is from a suicide attempt, which made him realise he doesn't want to die, so it was a bit of a turning point of his.
Even tho he doesn't want to die, he sucks at self-preservation, often ingnoring his injuries, old and new ones, forgetting
to take care of his basic needs, and lots of self-sabotage, both intentional and not. He also struggles a bit with addictions.
His friends are helping him fix those, but it's slow-going, especially since he tries hard to not let them see how bad it actually is with him.
