Doctor Gills is a somewhat flabby catfish that always appears dead and lifeless, yet never rots or smells.  His eyes have been sewn shut upon Gill's own request, considering them "unsettling to have to look into".  His pectoral and anal fins are unique in that they are more flesh and muscle and bone than actual fin-like material.  It has been suggested that in 'life', Doctor Gills might have used them to scoot himself around on land when he had to.  One of his most prominant features are his four whiskers - a trait all catfish share, but Doctor Gills takes great pride in.

Doctor Gills is a tan-brown fish with dark splotches above and a white underbelly beneath.  His dorsal and caudal fins are orange, yellow, and red while his pectoral and anal fins follow the patterns and colour of the rest of his body.  It's unknown to most what colour Doctor Gill's eyes actually are, since they're sewn perminantly shut.

The Doctor's two accessories are a hook and a hat.  The hook is perhaps the more interesting accessory, stabbing through the base of his tail and lodged there perminantly.  The story behind this strange feature is unknown, as he was like this before coming through the Gate.  Whatever the case, there's been no effort to remove the hook as it hasn't caused any harm to the fish's already dead body.  His hat, on the other hand, is something Doctor Gills himself insists his body wear.  He thinks it makes him look more dignified, and it seperates him from being considered a simple cadaver ready to be buried or burned.
