Billy works in the Academy's infirmary as a hospital clown. This means he works there as a Clown Doctor to tend to the young Companion's psycho-social needs i.e. help them deal with anxiety, sadness and anger. It can entail helping the children adapt to their surroundings or distract from and demystify painful or frightening procedures. But mostly, he plans to help them forget their illness and their stress for a moment. Billy wants to make the kids laugh, let them play and remember what it's like to just be a normal kid and not someone with an illness. He uses a mixture of clowning, magic, storytelling and music to make that happen. His favorite thing to do is creating balloon animals. Billy is also specifically good at juggling.
