The Silent Treater was a doctor on Alternia. At least to the higher castes. To lowbloods, they were a host of items and collections for the Signless. Their death was marked when the Empress caught word of this. Who told her, how she learned is completely a mystery. And nobody really knows how she truly died. Her body was never found.
