I don't know what his deal is either.
He's prone to erythrocytosis, which got worse when he started HRT. His wings, tails, and halo are made of his own blood. Extruding them, or "projecting," helps him manage his disease. If he uses his powers too much, he can pass out from blood loss (this was more common pre-HRT). His projected blood can form drops that he can give life, similar to nebula avatars. "Star avatars" are not known.
