He is able to transfer injuries from one person to the other but he has to act like the "transporter" and transfer the injuries from one person, to himself, then to the other person.

When in battle, he won't really attack but will let the other person attack him first and when he gets injured enough, he will run towards the attacker and hold them down to transfer the injuries.

Kyoya, at first, did not realize his quirk right away. That was until his friend, from elementary, got a scratch and when Kyoya tried to help them stand up, he fell down and looked at his knee in which he saw the same injury his friend had just had. When he looked over to his friend, his friend's wound was gone and was standing just fine. Kyoya went to the doctor and wanted to know if something had happened. He had learned it was his quirk that allowed him to transfer the injury onto himself.
After many practices, Kyoya was able to control his quirk so he can transfer at will rather than at random. He started to help people by transferring minor injuries to himself but he realized that people were only using him so they don't have to feel the pain anymore. Once realizing that it is so, he shut himself off from others.
Kyoya got his power from his mother. His mother fell ill to a grave illness while his father died as a hero when Kyoya was six. His mother soon passed away as well and Kyoya blamed himself for not being able to use his power to help his mother or his father. His brother, Akagi, joined the villains and Kyoya blamed himself for not being able to stop him. Feeling guilty for all the bad things that happened, Kyoya vowed to become much stronger and help people as well as convince Akagi to stop being a villain. Kyoya then joined the hero academy.
Hiro Yumeno Classmate/Crush
Yumeno is in the same class as Kyoya and wanted to befriend him. Although Kyoya tried to push him away, he eventually became best friends with Yumeno. Now, they're training together to become heroes.
Akagi Ichino Older Brother
Kyoya's brother has fallen to the dark side so Kyoya is trying his best to make Akagi realize that it isn't the doctors fault that their mother died or the heroes association's fault that their father died.
Yaguchi Ume Classmate/Enemy
Although Kyoya didn't care for any of his classmates except Yumeno. He noticed that Yumeno is starting to spend more time with Ume so he started to glare at Ume and thus began the hate-relationship.
