Sachiko is a second year at Iwatobi High who meets the swim team through the yearbook club when she is assigned to take their photo. Sister to Seijuro and Momotarou Mikoshiba. Intelligent genki girl who loves taking selfies.

Club: As part of the yearbook club, Sachiko formally meets the Iwatobi Swim Club when interviewing them about their relay race win in S1E9. She resigns from the club during her third year to focus on university entrance exams.
Animal: Clownfish, for her bright personality, social nature, and tendency towards clowning around.
Occupation: During S2 especially, Sachiko devotes her time to preparing for university entrance exams. She eventually attends medical school to specialize in pediatrics.
Fears: Being alone or left behind; not getting into her first-choice university
Personality:
+ An optimistic and upbeat girl, Sachiko firmly believes laughter is great medicine and strives to put a smile on friends’ faces whenever they’re down. Despite her tendency towards playful teasing, she knows when to take people seriously.
 + As an extrovert, Sachiko is unafraid of approaching others and can be a bit of a chatterbox. She also can’t help wearing her heart on her sleeve, which makes for very poor lying.
 + Sachiko prefers to be with other people and becomes lonely quickly if not occupied with work, especially when her brothers begin attending Samezuka Academy. 
 + Academically, Sachiko maintains excellent grades and favors science and mathematics. Whereas her brothers excel in athletics, Sachiko takes pride in her scholarship and likes to think of herself as Adorable Genius Sachiko!

+ Yuriko Kisaragi (mother): Yuriko is a physician specializing in pediatrics at Iwatobi Hospital. Although she works difficult hours, she is very dedicated to her daughter and even allows Sachiko to shadow her work. Yuriko divorced Sachiko’s biological father when Sachiko was young, so the two are very close and sometimes act more like friends than mother and daughter.
 + Masato Tanaka (stepfather): Sachiko’s eventual stepfather meets Yuriko after being transferred to Iwatobi Hospital. Sachiko pays Masato little mind at first, as up to that point Yuriko had never been in one relationship for very long. However, Masato’s proposal to Yuriko during S2 is very stressful for Sachiko, who fears this will alienate her from her mother. With guidance from her friends, she eventually accepts the change and becomes more flexible about facing the unknown and things she can’t control.
+ Seijuro Mikoshiba: Sachiko looks up to her older brother, particularly for his friendly competitive streak that they both share. Seijuro is equally proud of Sachiko’s academic record and her immortalization of the Samezuka team through copious snapshots. Sachiko may or may not have capitalized on Seijuro’s love for Gou by trading some cute photos for a quick buck…
+ Momotarou Mikoshiba: Perhaps because of their similarly energetic personalities, Sachiko and Momotarou frequently squabble – especially when his pet bugs invade her room – but beneath all the teasing, Sachiko really cares for her younger brother. Likewise, Momotarou enjoys playing pranks on Sachiko but also looks up to her. Momotarou often sneaks photos of himself onto Sachiko’s phone in the hopes that they will reach Gou.

+ Ayano Hamasaki (whitemistrose): Sachiko meets Ayano on their first day of high school during homeroom, when Sachiko notices Ayano’s “really cute!” hairbows and compliments them. The two social butterflies quickly become inseparable friends for the rest of high school and beyond, even rooming together for their university years in Tokyo. Sachiko confides in Ayano about everything, and she is the first to know about Sachiko’s poor performance on university practice exams.
+ Kaoru Hamasaki (krissehpoo): Through Ayano, Sachiko meets Kaoru, who she also quickly warms up to. Sachiko finds Kaoru is one of the few people who understands her perfectionism and the difficulties in balancing studies with a social life. She loves learning fun dance moves from the sisters, even if she can't always keep up
+ Haruka Nanase: Sachiko admires Haru’s impressive swimming but can’t help finding his cool, introverted exterior difficult to read and even harder to lift. One day, she’ll get a rise out of him! Although she pities his lack of motivation regarding life after high school, she doesn’t feel it’s her place to speak to him about it. Likewise, Haru gets along with Sachiko but doesn’t feel especially close to her.
+ Makoto Tachibana: Sachiko appreciates Makoto’s way with children and enjoys swapping funny anecdotes about their siblings. His warm, down-to-earth personality is very grounding for her expressive one and never fails to put her in a content mood. Despite being the same age, Sachiko’s honesty and chipper attitude remind him of his younger siblings.
+ Nagisa Hazuki: Nagisa is the first of the Style 5 that Sachiko meets due to being mutual friends with Ayano and Kaoru. Sachiko and Nagisa are like mirror images of one another and can be tiresome for anyone of unequal energy to be around. Although she loves him like a brother, she does chastise him for eating poorly (but can usually be tempted into a bite or two of his sweets…).
+ Rei Ryugazaki: Sachiko can get slightly competitive with Rei when it comes to academics but the two enjoy quizzing each other on different subjects. Generally, his dorky attitude can send her into peals of laughter, especially when it embarrasses Ayano. Regarding academics, Rei views her as a role model.
+ Gou Matsuoka: Sachiko supports Gou’s muscle appreciation 100% and compliments her smart position as team manager. The two can get a little carried away when it comes to taking swim team photos for the yearbook.

+ Rin Matsuoka: The Mikoshiba Curse is reversed in Sachiko, who exclaims Rin is “really cute!” upon their first meeting. Rin and Sachiko have a mutual respect for one another due to both being very goal-driven people. She enjoys teasing him about his significant other, which never fails to fluster him.
+ Aiichiro Nitori: Although Sachiko doesn’t interact much with Nitori, she sympathizes with his struggle and encourages him to chase his dreams whenever the opportunity arises.
+ Sousuke Yamazaki: Sachiko meets Sousuke the summer following 10th grade when he begins regularly visiting the hospital she volunteers at. Upon discovering Sousuke really does have a sense of humor beneath his serious exterior, the two become friends over the course of his daily hospital visits. They eventually exchange phone numbers and maintain a long distance friendship for the duration of Sousuke's enrollment at Tokitsu High, periodically meeting up during his holidays at home to catch up and run together. Sachiko’s distance from Sousuke’s past and competitive swimming allows Sousuke to confide in her about his fears regarding the future. In return, Sousuke consoles Sachiko when she struggles with preparing for university entrance exams.
