As for the code, I went to look for it and couldn't find it either. After digging around the coder's profile, in a reply to a comment they mention that they've taken freebies down for now, and while users who already have the code are free to use it, we should refrain from sharing it with others... (It's the most recent comment as of writing this, in case you want to take a look for yourself)
So yeah, sorry about that, I had no idea up until now. However, I do remember stumbling upon a different BNHA template recently, so I tried to find it! It's pretty close to what I currently have, just with fewer sections which you can add yourself if you really need them! (It might be even better than the one I used since I had to change the colors myself...) Here it is:

Alright, this might be somehwat long, please bear with me. HTML and CSS coding is by no means something I actually know how to do, I've just had the most minimal amount of classes related to it at school and then a TON of online tutorials.
Coding is such a diverse and wide-ranging activity that there is no way ANYONE can sum it up properly into a single reply, let alone someone as inexperienced as me. So, instead, I'd absolutely recommend that you read up on the basics instead (because everything else is really just googling stuff or experimenting yourself)!
