This is Pharragan's Lavia Forme, aka. Monster transformation.

He can transform at will with the help of an heirloom. This heirloom is a golden diadem imbued with a magical gem which gives him the ability to shapeshift. This becomes imbued into his monster self as he transforms and forms the Lavia forme's head and horns.
