The red bloches on his skin are not rashes that's his skin tone, he has red patches on his skin. 

His horns are about a foot and a few inches, the shakels around them are just decortaion. 

The beads on the one on the left one are for his brother.

He's the oldest sibling, he is a joking kinda guy but a super hot head and doesn't like when someone messes with who he considers family.
