Oh wow! What caused his current condition? Does he like his appearance or would he want a form that blends in whatever world he currently resides in?
His current condition was caused by dying and being dead. He was extremely powerful in life--on a scale of one to five he was probably a solid seven--so his ghost was immediately powerful enough to establish a power base in the Netherworld, which meant he caught the attention of the things stuck in the void-drain that really wanted to die properly and got empowered by them.
He likes his appearance as he is now more than he liked how he looked when he has meat on the bones--bones are much more gender-neutral than meat--and he has absolutely zero incentive to hide that he is King Shitkicker of (his chunk of) Fuck Mountain the rare times he deigns to venture into Creation. He wants to be seen for what he is, because if people are scared of him they're more inclined to obey.
