He can heal and create sicknesses with his magic (though he doesnt use the 2nd option) but he mainly uses his fire magic :D

Dr. Trousers is a medical doctor, specifically a surgeon, but he runs his own clinic he does more than just operate. 

Kera’tiv works as a surgeon in a backwater colony. They have actually been through medical school, but they don’t look it and don’t have the documentation to prove it. Nobody minds, though, because the colony is kind of desperate for good doctors.

Jadwiga is a physician in the 1860s. She's not quite sold on the germ theory yet but washes her apron and hands nonetheless for she's aware of the whole Ignaz Semmelweiss scandal and reckons that even if disease is caused by miasma then tidiness would prevent it's spread. :P
